Admission Procedure at Mamta Foundation
Admission criteria for children and women is as follows
Admission criteria for a child
- HIV +ve
- Total Orphan or single parent alive
- Single Parents
- Age between 3 yrs to 10 yrs ( Flexible upon case )
Admission criteria for women
- HIV +ve
- Orphan or destitute
- Willing to live as a mother of a child in the organization
- Age between 25 yrs to 40 yrs
Documents Required for Admission of a child
- Medical Documents
- HIV Tested report
- Recent CD 4 count
- Tuberculosis report / Chest x-ray
- Sonography ( abdominal )
General Documents
- Birth Certificate
- Death Certificate of parents / single parents
- School leaving Certificate
- Covering letter from NGO (if referred)
